This Notice describes how Second Opinion Physician / WalshDoc may use and disclose health information submitted through telemedicine, questionnaires, lab review, report preparation, and related services. This draft should be reviewed against your existing practice Notice of Privacy Practices and by appropriate legal/compliance counsel before broad live patient use.

1. Our privacy responsibilities

Second Opinion Physician / WalshDoc is committed to protecting the privacy and security of health information submitted for assessment, report preparation, lab review, follow-up, and related clinical or administrative purposes.

We are required to maintain the privacy and security of protected health information, provide this Notice of our legal duties and privacy practices, follow the terms of the Notice currently in effect, and notify affected individuals if a breach compromises the privacy or security of their information.

2. Information we may collect

Depending on the service, we may collect or receive information such as your name, date of birth, contact information, questionnaire responses, chief complaint, medical history, medication and supplement use, allergies, pregnancy status when applicable, laboratory values, reports, and follow-up information.

3. How information may be used

Information may be used for telemedicine assessment, clinician review, lab interpretation, report preparation, follow-up planning, administrative support, payment processing, quality improvement, and communication with you about your care or submitted information.

4. Your choices and rights

You may have rights to request access to your records, request corrections, request restrictions, request confidential communications, receive an accounting of certain disclosures, and obtain a copy of this Notice, subject to applicable law and practice policies.
