We are required to maintain the privacy and security of protected health information, provide this Notice of our legal duties and privacy practices, follow the terms of the Notice currently in effect, and notify affected individuals if a breach compromises the privacy or security of their information.
Depending on the service, we may collect or receive information such as your name, date of birth, contact information, questionnaire responses, chief complaint, medical history, medication and supplement use, allergies, pregnancy status when applicable, laboratory values, reports, and follow-up information.
We may use your information for treatment-related assessment, clinical review, report preparation, lab interpretation, supplement or nutrition strategy, follow-up planning, administrative operations, scheduling, billing/payment activities, quality improvement, and communication related to services you request.
Information may be submitted, transmitted, stored, and reviewed electronically. WalshDoc uses secure HTTPS transmission and restricted clinician access for submitted questionnaires and reports. The platform may use AWS hosting infrastructure under an active AWS Business Associate Addendum.
No electronic system can be guaranteed to be risk-free. Do not submit emergency information through WalshDoc. If you are having a medical emergency, call 911 or seek emergency care immediately.
We may use service providers, vendors, hosting providers, software systems, payment processors, email systems, or other business associates to support clinical, administrative, technical, or payment operations. When required, we use agreements intended to require appropriate protection of health information.
We may use or disclose information when required or permitted by law, including for public health, health oversight, legal proceedings, law enforcement, preventing serious threats to health or safety, workers’ compensation, or other legally authorized purposes.
Some uses or disclosures may require your written authorization. You may revoke an authorization in writing unless we have already acted in reliance on it. We do not condition care or services on authorizations except where allowed by law.

Requests for access, copies, amendments, restrictions, or confidential communications should be submitted to Second Opinion Physician using the contact information below. We may require verification of identity before releasing records or making changes.
You are responsible for providing accurate and complete information and for keeping your contact information current. You should not share your private questionnaire links or report access with unauthorized individuals. You should maintain an ongoing relationship with your primary care provider and appropriate specialists.
If you believe your privacy rights have been violated, you may contact Second Opinion Physician. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
We may update this Notice. Changes may apply to information we already have and information we receive in the future. The current version will be posted on this page.
